4.Decontamination | No decontamination supplies provided to handlers and early-entry workers. |
5. Application Records | Application records not accurately recording all of the necessary information. |
6. Worker Training | Workers are not trained before entering an area that was treated or under an REI within the last 30 days and every 12 months. |
7. Training Records | No training records are kept at the agricultural establishment for at least two years. |
8. Eye Flush | Not providing at least one pint of eyewash that can be immediately accessible when pesticide label requires eye protection. |
9. Personal Protective Equipment (PPE) | PPE not worn appropriately. |
10. Notification | Not providing oral and posting warnings when pesticide label requires both types of notifications. |
