Use this form to apply for a Premises Identification Number (PIN). There is no fee for this registration.

Instructions for completing this form:

2. Complete the required fields. Required fields are marked with an asterisk (*). Fields not marked with an asterisk are optional. 

3. Email the form to: ahealth@agr.wa.gov

4. Or, print and mail the completed form to:

5. Save a copy for your records.

Please allow 24-48 hours for processing. A premises information card, with premises number, will be emailed to you.
