All certified pesticide applicators and all persons applying pesticides to more than one acre of agricultural land in a calendar year must keep records of their pesticide applications for seven years. The WSDA requires unlicensed pesticide users to maintain records when performing applications to sites such as schools, daycares, apartment complexes, shopping centers, golf courses, and parks.

Pesticide application recordkeeping can occur in any format as long as the required information is included. If the WSDA requests the submission of pesticide application records you must use an official WSDA form. 

WSDA Approved Forms

Use the following WSDA approved forms for recordkeeping purposes. To have a modified pesticide application record form listed as a WSDA "approved version", fill out and submit the WSDA Pesticide Recordkeeping Form Approval Request.
