Food Storage Warehouse License
  What is the purpose of this license?
To allow the storage of any food other than grain, fruit or vegetables
  What activities require this license and who needs to get it?
Facilities where food is stored, kept, or held for distribution to wholesale facilities, retail outlets and restaurants
  How do I apply?
You apply by completing an application form.
  Do I need to include anything with my application?
Complete and send the Food Storage Warehouse application form along with a check or money order.
  Is the decision to approve dependent on anything besides the information in my application?
A WSDA Food Safety Compliance Specialist (FSCS) reviews all food storage areas of your facility. Before leaving the facility, the FSCS will leave a copy of the inspection report with you. The inspection report will note any areas that require correction. At that time the inspector will also inform you of your licensing status (approved or denied). All subsequent inspections are unannounced visits and will occur every six to twelve months.
  When will I receive my license?
Four to six weeks after you submit a completed application form.
  How long is it valid?
All Food Storage Warehouse licenses expire March 31st of every year.

  What is the appeal process?
Upon notice by the Department to deny, revoke, or suspend a license, a person may request a hearing under chapter 34.05 RCW.
