  What is the purpose of this license?
To regulate those companies distributing commercial feed.
  What activities require this license and who needs to get it?
Facilities that offer for sale, exchange or barter commercial feed, if not otherwise exempt.
  How do I apply?
You apply by completing an application form located here.
  How much will it cost?
$50 per year
  Do I need to include anything with my application?
Include the fee with the application.
  Is the decision to approve dependent on anything besides the information in my application?
No. But after your license is approved, you must report tonnage on a semi-annual basis.
  When will I receive it?
Approximately 15 days after the Department receives a completed application.
  How long is it valid?
All Commercial Feed Licenses expire on June 30th each year.

Request a hearing under Chapter 34.05 RCW [external links], the Administrative Procedures Act.
