Pesticide/Structural Pest Inspector 2012 License Renewal
All licenses except the Commercial
Applicator license can be renewed on-line. The on-line option is expected to be available to Commercial Applicators
by Spring 2012.
- Non-Commercial Applicators – If you would like to renew on-line, click here. Otherwise, read the instructions below and, if eligible for renewal, complete the
renewal form and
submit it along with your license fee by mail to the address at the top of the form.
- Commercial Applicators – Complete Form A; complete
Form B to report company-employed Commercial Operators and
multiple business locations. Make sure that WSDA has current financial coverage
information by searching on your company
record and viewing the listed insurance expiration date. If expired, your agent will need to submit a completed
Financial Responsibility Insurance Certificate. Submit all necessary forms and payment by mail to the address at the top
of Form A.
Instructions: The on-line renewal process will verify if you are eligible to renew. If renewing
through the mail, you should only complete a renewal form for licenses that qualify for renewal. To check the status of
your license(s), go to the License Search page and enter your license number or name. Any license listed that is in an
active recertification cycle can be renewed. If your recertification status is "Pending Recertification" or
“Failed to Recertify,” DO NOT complete a renewal form. Please call Pesticide Licensing staff at 877.301.4555 for further
instructions. If you do qualify for renewal, you must either sign the "Penalty Fee Exemption Statement" declaring that you
have not operated as a
licensee since Jan. 1 or you must pay the late fee.
