WSDA Pesticide License Application Forms
for Reciprocal Licensees

If you are applying for a Washington pesticide license based on a license from another entity that WSDA has approved for reciprocal licensing, you may complete the Reciprocal Pesticide License Request form and one of the two WSDA pesticide applications below depending on the type of license you need. Submit the completed application along with a copy of your current pesticide license and the applicable fee.

• Commercial Applicator – use this form if you are the head of a business that commercially applies pesticides.

Instructions: Complete Form A; complete Form B to report company-employed Commercial Operators and multiple business locations. In addition, your agent must submit proof of financial coverage on a Financial Responsibility Insurance Certificate or Surety Bond form.

• Non-Commercial Applicator – use this form for all license types EXCEPT the Commercial Applicator license.
