Forms

Forms are available by Topic, Name and Number. Please click the corresponding button in the left menu.

To view PDF files, download Acrobat Reader.

Instructions for Using Fillable Forms

Adobe Acrobat Reader (version 4 or higher) can be used to view, complete and print fillable forms. This free software can be downloaded from the Adobe Web site. Acrobat Reader does not allow you to save information entered on completed forms. Other software (e.g. Acrobat Standard, Adobe Acrobat Professional) is required to save a completed form.


How to Complete Fillable Forms

  1. Click on the link to open the fillable form in Acrobat Reader.
  2. To enter text in a form field: Click on the “Select Text” button and position the mouse pointer with the I-beam over a form field, left click, and enter text.
  3. Press the TAB key to accept text entered (or use the mouse) and go to the next field. Continue filling out the form.
  4. Press SHIFT+TAB to accept text entered and go to the previous field.
  5. RETURN takes you down another line in a multi-line field.
  6. The arrow pointer allows you to select a check box, radio button, or an item from a list.
  7. Use either the scroll bar on the right side of the Internet browser window or the hand tool in the Acrobat toolbar menu to view other areas of forms.

Printing Fillable Forms

To print a completed form, click on the PRINT button on the Acrobat toolbar that instead of the PRINT icon on the Web browser toolbar. You will not be able to print the form from the file menu on the top of the screen. Manually sign the fillable form (if applicable).

Tips on Solving Printing Troubles

  1. Download the file you want to print to your computer instead of printing it from inside a browser window. You can download a fillable PDF form to your computer for use later by right clicking on the form title link then select "Save Target As" from the dropdown options.
  2. Open the downloaded file in Acrobat Reader and print the file.
  3. Set the dots per inch (dpi) for printing to 300 instead of a higher resolution.
  4. Change the print quality from high to low.
  5. Print one or two pages at a time instead of the entire file.
