Forms for Fairs Officials

Please note: The following forms are only for use by fair management officials or other authorized personnel.  Some functionality has been restored to the Community Fair Funding form and the two County/Area Fair funding forms, but they can only be saved on your computer if you have Acrobat Standard or Professional. You will need to print the completed forms if you do not have this software. We hope to restore interactive form functionality to all forms soon.  In the meantime, we have provided links to static PDF forms where fillable forms are not available.
